What is an assessment?
This is a process that a licensed or an unlicensed person completes to document the medical care, personal care, psychosocial and spiritual needs and preferences of an older adult.
The assessment is a means for gathering information to be used by a caregiver or health care provider to extend care in an effective and meaningful way.
An assessment is also intended to document and to highlight special information and tasks that affect the care and interaction a caregiver or provider has with a senior.
